Hello!
My name is Inez and I am one of your local Certified Community Health Workers.
If you have IEHP, Molina or LA Care you have come to the right place!
Our passion is finding the right resources to fit your needs.
After enduring 5 heart attacks and a quadruple bypass, I understand the importance of finding resources, choosing the right doctors and having the right health plan.​
We find it hard to look for help because we are misinformed or just don't know who to ask. I became a Certified Community Health Worker to help those in need.
We look forward to serving you!
As a Community Health Workers:
- We have a trusted relationship with the people we serve.
- We can help you transfer your Medi-Cal to your new County of Residence.
- We can help you enroll into Medi-Cal, IEHP, Molina and LA Care.
- We help complete applications for health plans, social security, insurance, health services, immigration forms and more!
- We can interpret and translate between health care service providers and patients as well as USCIS and applicants.
( Some of us are bilingual English and Spanish)
- We can help you learn how to find and access resources like quality care, health information, health insurance, life insurance, Citizenship, immigration, housing, food, and more!
How much does this cost? Nothing.
If you have IEHP, Molina, or LA Care with your Medi-Cal you qualify!
What do you need?
Your IEHP Member card. Yes, That's it.
Misplaced your card? No problem! We just need your Full Name and Date of Birth to check for eligiblity.
(Plus we can help you replace your card too!)
